The Louisiana Department of Revenue has made the most commonly requested tax forms available in electronic format. Current forms include the current year forms and forms for the previous five years. Several of these forms are Fill-In forms which makes it possible for you to enter information while the form is displayed and then print the completed form. Fill-In forms are denoted with a pencil icon.
For liability reasons, scripts have been added to our tax forms to prevent the data from lingering on public access computers. If you need a form that does not have these scripts included in them, you may call Communications Section at 219-2760 and request the electronic form/s without the scripts.
