How can I get a copy of my Louisiana tax return or tax records?
If you have a
LaTAP account, you may be able to
view and print available copies of your tax returns and tax records at no cost by logging in to your account.
If the document you need is not available through LaTAP, you may request a copy from the Louisiana Department of Revenue (LDR) by completing Form R-7004, Tax Information Disclosure Authorization.
Fees for copies requested from LDR:
$15 for each copy of a tax return or other tax document, per tax year or tax period.
$25 for each certified copy, per tax year or tax period.
Important
Research fees are required even if the requested return or document cannot be located.
Payment must be made by check, cashier's check, or money order payable to the Louisiana Department of Revenue.
Cash is not accepted.
