What to do if you received a Notice of Unclaimed Property letter

You received a Notice of Unclaimed Property letter from the Louisiana Department of Revenue because our records indicate a state tax refund check issued to you has not been cashed.

What should you do? Enter the appropriate information on the subsequent pages of LaTAP. Visit Unclaimed Refund. This will help us to reissue your refund check. If approved, you can expect to receive your replacement refund check by October 15.

For a name change, please attach any of the following applicable documents:

  • Social Security card
  • Driver’s license
  • Divorce decree
  • Marriage license

If the replacement refund check is being requested on behalf of a deceased taxpayer, please submit both of the following documents:
