What to Bring

Boat Registrations

When a boat is sold from Individual to Individual, bring:

  • Valid picture identification
  • A completed LDR Form R-1331, Boat Registration Tax Payment Certification
  • Notarized bill of sale between both parties
  • Previous owner’s boat registration/title
  • For payment of taxes:
    • Baton Rouge: Cash, check or money order.
    • All other LDR regional offices: Check or money order.

When a boat is sold from Dealer to Individual, bring:

Commercial Fisherman

When you need a commercial fisherman exemption, bring:

  • Valid picture identification
  • Your current federal income tax return along with all supporting Schedules
  • Valid Louisiana vessel license from the Department of Wildlife & Fisheries
  • Proof of vessel ownership
  • Your current commercial fishing license
  • Your completed Form R-1334, Commercial Fisherman Louisiana Sales Tax Exemption Application
  • First year applicants for the commercial fisherman exemption will need to provide a Notarized statement declaring no income was derived from commercial fishing for the year

Requesting Copies of Return or Other Documents

Gaming Clearance

  • Valid picture identification
  • Your completed Individual Consent To Disclose Federal Tax Information / Clearance Form. This form is part of your Louisiana Gaming Control packet which you can get from the Office of State Police (preferably in blue ink).
  • What does LDR verify?

IFTA Application or Decals

New IFTA Account

  • Valid picture identification
  • Your Federal Account Number (FEIN) if your account will be setup as a LLC
  • Your completed LDR Form R-5678-L, Decal Request
  • Payment for the application fee ($35) and the decal set ($2):
    • Baton Rouge: Cash, check or money order.
    • All other LDR regional offices: Check or money order.

Existing IFTA Account Decal Request

  • Valid picture identification
  • Your Louisiana Department of Revenue account number
  • Your completed LDR Form R-5679, Decal Request
  • Payment for the decal ($2):
    • Baton Rouge: Cash, check or money order.
    • All other LDR regional offices: Check or money order.
